Wedding Shower Order Form: Requesting Borrowed Copy

Does anyone have a copy of the Wedding Shower Order form? I know I have seen it posted before, it has a space for the guest to put the brides items in one section and then the items that they are ordering for themselves in the other section. If anyone has this form, I'd appreciate you letting me "borrow" it. :blushing:

if you go under files and then look for order forms, you will find it. I can't remember what it is called.

Yeah, I've tried looking for it but it's not coming up, maybe I'm typing in the wrong search words. I will keep trying.

I am new to PC, but have booked a Wedding Shower show. How can I personalize a form such as this? Do I need to purchase software or does PC offer resources to create my own?
