What Do I Put: Company/Business Affiliation Info?

It is asking me for my company or business affiliation, address and all of that fun stuff, what do i put? Do I put my personal into or Pampered Chef's. If I do put PC's info do I use them as my employer too? These may seem like dumb questions but I wanted to make sure before I do anything

Search for national teams, and Pampered Chef should come up. There are instructions on CC with the HWC info.
