I made up this Product sheet for my daughter's cheerleading squad. Thought I would share it with the rest of you. I printed it on white card stock paper and on the other side I printed the order form. I'm at work and don't have the order form on my computer. (I'm supposed to be working but you see what I'm doing instead). I'll post the order form when I get home later today. Hope some of you can use this idea. It took a while to get it done but it's finished. I added the sales tax to the prices and put a column on the order form for the shipping cost.

Let me know if this is helpful to anyone or if any of you see something I should change or shouldn't have done!!
