I always put C/O "consultant name" in line 2 so they know to leave it for you.
Example - it will read:
Jane Doe (Host)
c/o Susie Consultant
123 First Avenue
Beverly Hills, 90210
You have to put it in the first box though if she won't list her address.

Shows hosted by you or anyone living with you do not get booking benefits on shows that are booked off of them and the products purchased with the host discount are not commissionable.
