Need a Gift Certificate Template? Not Sure What to Include?

Does anyone have a template for making a gift certificate or a sample gift certificate? I'm not sure what to include on it? Should I include an expiry date?
 
The company provides GCs for us to use
 
I know someone made some up and they are somewhere, I am assuming that you are wanting to make up a gift certificate to be redeemed through you is that right

Yes. We don't have them offered through the company in Canada but I would like to offer some for the holidays.
 
Did you try clicking on files and searching on "certificate"? I found a bunch of files there not sure if any of them are what you are looking for though.
 
Here are a few from this site...Thes are a few I have gotten from this site and I use when a regular GC is not what I need. I have used these when the guest wants a "dummy" GC to put in a birthday card. I also use them for hosts, incentives, etc.

They are in WORD and easily manipulated to fit your needs.

Thank you for the examples. I am unable to open them. I tried to save them and then open it, but no success. Can you post them in Wordpad? I have Microsoft Office.
