By the way I made stickers/labels with this information so I don't have to write it out all the time and saved it on my computer in my PC folder I made.
 
I wrote it in my address book I keep in my purse, so that way if I have to take something to UPS or FedEx to ship, I can just read it to the clerk. It worked like a charm the time I went to UPS.
