I need your feedback on this...it's a work in progress, but I needed one. I know it's in publisher, but it's so much easier to use than word. I will try to put it in PDF too!

I agree with Heather--it looks great! Thanks for putting it as a PDF so I could open it. I have Word. If you do change anything would you repost. I personally think it is fine unless I am too tired to see anything else.
