Director Looking for Team Member Q&A Form - Help Appreciated!

I'm looking for a form I saw probably around this time last year. It's basically a form for team members asking "What do You Want from your Business and How Much Help do you want from me?" Type of Q&A. Y'all know what I'm referring to? I'd love a copy if you have something like this,

I think there was a drawing slip type form in a recent directory as a mtg. handout.

Here is what my Dir. used to give out during NCT.
(I have this in word, but won't let me attach--invalid file...someone PLEASE explain to me why we get these errors on Word Docs.?)

Maybe the error is due to using the newer version of word?

Yes try saving it in a Word 2003-2007 format (When you go to Save As, you can choose the format from the drop-down menu). I would love this in Word!
