Besides putting my consultant info on these I put on the OO order form the host name, who the check should be made payable for that show and a little thank-you for filling out the form completely so I can serve you better in the future note on top.
I then click download and save and then save it as a Pdf to my PC folder on my computer so I can print it from there and not get the www stuff on the bottom of the page when I print it.
