How much should customers add for shipping when ordering from out of state?

I have a host who has customers out of state that want to order from her show and ship it directly to them. What is the best way for me to tell her how much they should add for shipping?
 
Direct shipping is on the back of the paper order forms.

She has the outside order form so that doesn't have it on it. Is there a chart or something online I can send her to use or to tell her customers?
 
The second page of the outside order form has the direct shipping charges.
