Need an Order Order Form for a Baby Shower.

I was woundering if i should just use my show orde forms or use the one like from the wedding order forms that say buy fr couple or or for your self. which ones?

I'd certainly use a form that allows people to order for themselves as well as for the mommy-to-be.
