Can anyone help me edit this bulletin board flier to make it usable?

I'm hoping someone can help me with a problem. I found a bulletin board flier on a board (can not remeber which one and can not find it in any of the document areas). Anyway, I have tried to edit it with my name and number but have had no luck. I really like the layout of this flier, but if I can't edit it, it does me no good. Does anyone have one they could post? TIA
