Need Help with a Last-Minute Wedding Gift Certificate from Pampered Chef?

Hey all, I was wondering if one of you could help me out! I'm wanting to give a friend a last-minute wedding present of a $50 "gift certificate" to PC, through me. I don't have time to buy one through HO though, obviously. What I want is a "gift certificate" with an order form that she can mail to me, or call in to me, similar to the gift certificates offered by PC. Does anyone have anything like this already made up? I'm leaving this afternoon and I may not have time to make one myself.
